Once you’ve paid your tuition, you will need to complete three forms in order to finalize your enrollment in The Bird Dog Program. Please follow these simple steps:

- Step 1: Download the forms in Word DOC or PDF format.
- Step 2: Print the three forms.
- Step 3: Complete the forms.
- Step 4: Scan the completed documents.
- Step 5: Send the documents via email to Kristine@TheBirdDogProgram.net -OR- Click Here to Upload and Submit.
